ABSTRACT

Xiaoyaowan has the effects of soothing the liver, invigorating the spleen, regulating menstruation, and nourishing blood. It is often used to treat the distending pain of the chest and hypochondriac, loss of appetite, dizziness, and irregular menstruation caused by liver depression and spleen deficiency. The formula originated from the Sinisan of Treatise on Cold Damage(《伤寒论》) in the Han dynasty and was officially formulated in the Song dynasty's Taiping Royal Prescriptions(《太平惠民合计局方》). In the Ming and Qing dynasties, it was developed into more comprehensive formulas such as modified Bazhentang, modified Xiaoyaowan, and other formulas. In recent years, Xiaoyaowan has become a classic formula for treating many symptoms of liver depression. The Pharmacopoeia of the People's Republic of China(《中华人民共和国药典》), 2020 edition, records Xiaoyaowan and modified Xiaoyaowan. Modern clinical research has further expanded the therapeutic range of this formula. Through literature research, it is found that there are certain reports on the clinical application and quality analysis of Xiaoyaowan, but the relevant literature lacks collation so far. Therefore, relevant literature was consulted and sorted out. The paper summarized the treatment of mammary hyperplasia, depression, irregular menstruation, melasma, menopausal syndrome, and polycystic ovary syndrome with Xiaoyaowan, as well as the quality analysis of paeoniflorin, ferulic acid, and glycyrrhizinic acid of Xiaoyaowan. It is expected to lay a foundation for further research on clinical application, pharmacodynamic mechanism, and quality control of this classic formula.

ABSTRACT

Objective:To evaluate the clinical effect of treatment of activating blood and removing blood stasis, invigorating the spleen and soothing the liver for the patients with gastric collateral stasis syndrome and chronic atrophic gastritis (CAG).Methods:Randomized controlled trial. A total of 68 CAG patients admitted to the Huairou Hospital of Traditional Chinese Medicine from January 2018 to January 2021 who met the selection criteria were divided into 2 groups according to the random number table method, with 34 in each group. The control group received conventional western medicine treatment, such as inhibition of acid, protecting the gastric mucosa, and the observation group was treated with Traditional Chinese Medicine (TCM) herbal prescription of activating blood and removing blood stasis, invigorating the spleen and soothing the liver. Both groups were treated for 12 weeks. TCM symptom scores were performed before and after treatment. The serum level of pepsinogen Ⅰ(PG Ⅰ), pepsinogen Ⅱ (PGⅡ) were detected by ELISA, and the PG Ⅰ/PG Ⅱ ratio was calculated. Gastroscopic biopsy was performed to observe the changes of intestinal metaplasia of gastric mucosa and glandular atrophy, and to evaluate the clinical efficacy. Results:The total responsive rate was 85.3% (29/34) in the study group and 58.8% (20/34) in the control group. There was significant difference between the two groups ( χ2=9.35, P=0.030). After treatment, the scores of stomachache, fullness of feeling in the observation group were significantly lower than those in the control group ( t=2.97, 3.80, P<0.05). After treatment, the level of serum PG Ⅰ[(76.21 ± 17.35) mg/L vs. (66.8 ± 18.77) mg/L, t=2.15] and PG Ⅰ/PG Ⅱ [(4.67 ± 0.99) vs. (3.90± 1.25), t=2.81] in the study group were significantly higher than those in the control group ( P<0.05), and PG Ⅱ [(16.36 ± 1.85) mg/L vs. (17.42 ± 2.05) mg/L, t=2.24] was significantly lower than that of the control group ( P<0.05). After treatment, intestinal metaplasia and glandular atrophy was significantly more improved or reversed than those in the control group ( χ2=20.67,9.33, P<0.05). Conclusion:The methods of activating blood and removing blood stasis, invigorating the spleen and soothing the liver can reverse the precancerous lesions of patients with gastric collateral stasis syndrome of chronic atrophic gastritis and have a good prognosis.

ABSTRACT

Bronchiectasis is characterized by a “vicious cycle” involving compromised host defense, mpaired clearance of airway mucus, bacterial colonization, infections, and inflammation, leading to frequent acute exacerbations and diminished quality of life. Based on the hypothesis of “vicious circle”, this paper explores the treatment of bronchiectasis by traditional Chinese medicine (TCM) from three aspects. Firstly, dissipating excessive “phlegm” secretion and enhancing airway clearance form the foundation of the treatment, aiming to improve the condition of mucus hypersecretion. Secondly, invigorating spleen for strengthening vital energy can improve the function of immune system and reduce recurrent infections and acute attacks. Lastly, clearing heat and purging the lung can alleviate infection and inflammatory damage. By employing these methods, TCM can disrupt the progression of the “vicious circle”, delaying disease advancement, minimizing acute exacerbations, and improving the quality of life for patients.

ABSTRACT

Gastric ''inflammation-cancer'' transformation stars from inflammation and ends as gastric cancer (GC), and the pathogenesis is still unclear. In China, GC features high morbidity and mortality and poor prognosis, influencing the quality of life and physical and mental health of patients. Therefore, it is of great significance to construct the prevention and treatment system for GC. Chronic atrophic gastritis (CAG) plays a key role in the occurrence, development, and outcome of gastric ''inflammation-cancer'' transformation. Modern therapies for CAG generally aim at eliminating causes and alleviating clinical symptoms, which show satisfactory short-term efficacy, but the reverse and recurrence are common. Based on the holistic view, syndrome differentiation-based treatment, and the ''inflammation-cancer'' transformation in modern medicine, traditional Chinese medicine emphasizes both prevention and treatment, with individualized therapies for CAG and GC to control the transformation. According to the pathogenesis of CAG-asthenia in origin and sthenia in superficiality and deficiency-excess in complexity, this study proposed the theory of spleen deficiency and pathogen stagnation in CAG, and believed spleen deficiency, pathogen, and stagnation are respectively the root cause of, the main factor of, and the key to ''inflammation-cancer'' transformation, respectively. Spleen deficiency and pathogen stagnation are closely related to the process of the transformation. For the treatment, the spleen-invigorating and pathogen-eliminating method should be used for invigorating the spleen to consolidate original Qi, improve the blood supply in stomach, and regulate immunity, and eliminating the pathogen to relieve stagnation, reduce the occurrence of non-controllable inflammation, and improve inflammatory micro-environment. As a result, the gastric inflammation is controlled at the early stage and the gastric ''inflammation-cancer'' transformation is blocked. The gastric mucosal lesions are blocked, delayed, or even reversed. This study provides a new idea in clinical diagnosis and treatment of CAG and in the prevention of GC.

ABSTRACT

The aim of the study is to explore the application of simultaneous treatment of spleen and kidney in the treatment of sarcopenia by sorting TCM literature and related research. It is believed that the location of sarcopenia is in the muscles and sinews, and the main pathological changes are located in the spleen and kidney, which is closely related to the kidney. The pathogenesis is deficiency of the spleen and kidney, muscle deprived of nourishment, and the treatment should focus on invigorating spleen and tonifying kidney. The mechanism of invigorating spleen and tonifying kidney in the treatment of sarcopenia may be related to improving mitochondrial function, reducing inflammatory factors, promoting insulin secretion and repairing muscle fibers.

ABSTRACT

ObjectiveTo explore the effects of different treatment methods of "soothing liver, invigorating spleen, soothing liver and invigorating spleen, soothing liver first and then soothing liver and invigorating spleen, as well as invigorating spleen first and then soothing liver and invigorating spleen" on liver depression combined with liver injury in rats and their action mechanisms. MethodA six-week rat model of liver depression combined with liver injury was established by restraint stress and subcutaneous injection of carbon tetrachloride (CCl4, 5.89 g·kg-1, once every three days). At the same time, the drugs were given by gavage. Forty-eight male SD rats of clean grade were randomly divided into eight groups, namely the normal group, model group, bicyclol (0.2 g·kg-1) group, Sinisan (4.32 g·kg-1) group, Liu Junzitang (9.26 g·kg-1) group, Chaishao Liu Junzitang A (Chai A, soothing liver and invigorating spleen,13.57 g·kg-1) group, Chaishao Liu Junzitang B (Chai B, soothing liver first and then soothing liver and invigorating spleen, 13.57 g·kg-1) group, and Chaishao Liu Junzitang C (Chai C, invigorating spleen first and then soothing liver and invigorating spleen, 13.57 g·kg-1) group, with six rats in each group. The pathological changes in liver and colon tissues of each group were observed under light microscope and electron microscope. The serum biochemical indexes of the liver were detected using an automatic biochemical analyzer. The relative mRNA expression levels of Takeda G protein-coupled receptor 5 (TGR5) and intestinal mucosal zona occluden-1 (ZO-1), Occludin, and Claudin-1 in the liver and colon were detected by reverse-transcription polymerase chain reaction (RT-PCR). The positive expression rate of proliferating cell nuclear antigen (PCNA) in the colon was detected by immunohistochemistry. ResultCompared with normal group, the model group exhibited significantly elevated serum alkaline phosphatase (ALP), alanine aminotransferase (ALT), aspartate aminotransferase (AST), total bilirubin (TBIL), and direct bilirubin (DBIL) (P<0.01), lowered TGR5 mRNA expression in liver tissue, up-regulated TGR5 mRNA expression in the colon tissue (P<0.05,P<0.01), and down-regulated ZO-1, Occludin, and tight junction protein-1 (Claudin-1) mRNA expression and PCNA in the colon tissue (P<0.01). Compared with the model group, bicyclol and Chai C remarkably decreased the levels of serum ALP, ALT, AST, TBIL, and DBIL (P<0.05,P<0.01), while Liu Junzitang, Chai A, Chai B, and Chai C significantly up-regulated the TGR5 mRNA expression in the liver and down-regulated its expression in the colon (P<0.01). Bicyclol, Chai A, Chai B, and Chai C enhanced the ZO-1 and Claudin-1 mRNA expression in the colon (P<0.05,P<0.01). Bicyclol, Sinisan, and Chai C increased PCNA expression (P<0.01). The comparison with the Chai C group showed that the TGR5 mRNA expression in the liver and ZO-1 mRNA expression in the colon of the bicyclol and Sinisan groups were lower, whereas the TGR5 mRNA expression in the colon was higher (P<0.01). However, the PCNA expression in the colon of the Liu Junzitang and Chai B groups declined significantly (P<0.05). ConclusionIn the presence of liver injury, invigorating spleen first helps to relieve the liver injury, and the efficacy of "spleen-invigorating" therapy in increasing the intestinal mucosal tight junction proteins and improving the gastrointestinal function is related to its activation of TGR5 to improve the intestinal mucosal barrier function, promote the renewal of intestinal stem cells, and drive the regeneration after injury.

ABSTRACT

As the first line of defense between the intestinal environment and the outside world, the intestinal mucosal barrier is essential for maintaining the intestinal homeostasis. The intestinal mucosal barrier injury will change the intestinal permeability and allow bacterial translocation and the entry of endotoxins into blood, thus triggering a series of inflammatory responses, followed by the injury of related tissues and the aggravation of primary diseases. The spleen, the acquired foundation, is responsible for maintaining the internal and external balance of the body and resisting external evils. Its physiological function is similar to that of the intestinal mucosal barrier. Spleen deficiency easily leads to intestinal mucosal barrier dysfunction. Therefore, replenishing Qi, invigorating spleen, and restoring the efficacy of spleen and stomach qi in defensing and governing transportation and transformation are the keys to prevent and treat intestinal mucosal barrier injury. In recent years, studies have shown that the spleen-invigorating Chinese medicinals repair the intestinal mucosal injury by promoting the expression of intestinal epithelial tight junction proteins, regulating the intestinal immune function, microbial flora, and metabolites, and supplementing the intestinal nutrition, enabling them to gradually become a research hotspot. After reviewing the relevant articles published in China and abroad, this paper expounded the common syndrome types of traditional Chinese medicine (TCM), the changes in intestinal mucosal barrier induced by spleen deficiency, the repairing effects of spleen-invigorating Chinese medicinals on intestinal mucosal barrier injury, in order to provide some clues for the research on the treatment of intestinal mucosal barrier dysfunction-related diseases with spleen-invigorating Chinese medicinals.

ABSTRACT

According to the theory of traditional Chinese medicine (TCM), spleen and stomach are the most important visceral system to maintain human life and health, and healthy spleen and stomach can help to nourish the primordial qi, so as to maintain sufficient vitality of the body. Therefore, invigorating the spleen plays an important role in the prevention and recovery of human diseases. In this paper, the correlation between the theory of invigorating the spleen of TCM and the function of modern health care was discussed, and the relationship between spleen invigorating and digestive tract protection, immunomodulation, anti-oxidation, relieving physical fatigue, helping to reduce fat and lose weight, improving sleep and other modern health care functions and its mechanism were systematically summarized and analyzed. The mechanism and material basis of important spleen-invigorating Chinese medicinal materials such as Astragalus membranaceus, Poria cocos, Dioscorea opposita, Euryale ferox, Atractylodes macrocephala and Polygonatum sibiricum and classic recipe of invigorating spleen such as Guipi Decoction, Sijunzi Decoction, Buzhong Yiqi Decoction and Lizhong Decoction were briefly reviewed, in order to provide some new ideas and methods for the development and utilization of spleen-invigorating medicine and food resources in TCM and the research and development of TCM health food.

ABSTRACT

Objective: To observe the improvement of androgen deficiency syndrome after endocrine therapy by "Jianpi Lishi Huayu Method" for advanced prostate cancer and its clinical effect. Methods: A total of 60 patients with advanced prostate cancer were randomly divided into observation group (30 cases) and control group (30 cases). The control group was treated with intermittent endocrine therapy (androgen blocking). The treatment group was treated with "Jianpi Lishi Huayu Decoction" on the basis of the control group. The end point of the study was the time to develop castration-resistant prostate cancer. ISS scale scores, disease progression-free survival, PSA, serum testosterone, I-PSS and TCM clinical symptom scores were compared between the two groups. Results: The median progression-free survival time of the patients in the observation group was 23 months, and there was no significant difference between the two groups. After treatment, the total ISS score, physical symptoms score, vasomotor symptoms score and psychosocial symptoms score in the observation group were significantly lower than those before treatment, and lower than those in the control group (P < 0.05). After treatment, the effective rate of ISS score in the observation group (73.33%) was significantly higher than that in the control group (16.67%); The PSA level in both groups decreased significantly after treatment, and the PSA level in the observation group (2.13 ± 0.58) ng/mL was better than that in the control group (5.30 ± 1.40) ng/mL (P < 0.05); There was no significant difference in serum testosterone between the two groups after treatment. After treatment, the I-PSS score and TCM clinical symptom score of the observation group and the control group were improved, and the observation group was better than the control group (P < 0.05). After treatment, the effective rate of the I-PSS score of the observation group (96.67%) and the improvement rate of TCM clinical symptom score (76.67%) were better than those of the control group (73.33%, 26.67%). Conclusion: On one hand, "invigorating spleen, draining dampness and removing blood stasis" method combined with endocrine therapy can prolong the time for hormone-dependent prostate cancer to develop into castrated resistant prostate cancer, improve clinical efficacy, reduce the level of tumor markers, stabilize the level of testosterone, and improve some androgen deficiency syndrome after endocrine therapy, especially in ovariectomized resistant prostate cancer. It has obvious advantages in improving physical symptoms, vasomotor symptoms and psycho-psychological symptoms. It can obviously alleviate adverse reactions after endocrine therapy. On the other hand, it can better improve clinical symptoms, reduce I-PSS scores and TCM clinical symptoms scores, improve the quality of life of patients, and give full play to the role of reducing TCM toxicity and increasing synergism efficiency.

ABSTRACT

Wandaitang, which is one of classical traditional Chinese medicine(TCM) prescriptions, is derived from Collected Exegesis of Recipes of Fu Qingzhu' s Obstetrics and Gynecology. It is commonly used in modern clinical practice, and included in the Catalogue of Ancient Classical Prescriptions (The First Batch). Collected Exegesis of Recipes of Fu Qingzhu' s Obstetrics and Gynecology and Chen Shi-duo' s Bianzhenglu have a complicated relationship. Both of them have another biography, named Nvke Xianfang and Bianzheng Qiwen. The composition of Wandaitang in the four books is slightly different, while the prescription's explanations and other records are almost the same. The research and development of Wandaitang should be based on the records of Collected Exegesis of Recipes of Fu Qingzhu's Obstetrics and Gynecology. Compared with other classical literatures, Collected Exegesis of Recipes of Fu Qingzhu's Obstetrics and Gynecology was published in a late period and less reproduced in other ancient books. To study the function of Wandaitang, we need to analyze the records in the original book. In addition, we need to make a multi-angle analysis by reference to the theory of TCM, the composition of drugs, the significance of compatibility, as well as the understanding of modern famous doctors, clinical reports and experimental studies in all aspects. The study found that the functions of Wandaitang were relatively concentrated, but with wide major functions involving internal medicine, surgery, gynecological, pediatric, andrological and other departments. According to the study, the authors believe that the functions of the classical TCM prescription of Wandaitang are invigorating spleen to eliminate dampness, dispersing the liver and rectifying Qi, and invigorating Yang. It can be used to treat leucorrhea, diarrhea, edema and stranguria with the syndromes of pale, languid, little food, loose stool and depression. Wandaitang can also be used to treat vaginitis, cervicitis, pelvic inflammatory disease, irritable bowel syndrome, chronic colitis, chronic nephritis, nephrotic syndrome and chronic prostatitis.

Objective To explore the effect of invigorating splcen and kidney methods on the pregnancy outcome of the intrauterine insemination (IUI) in infertile men with asthenospermia. Methods a total of 78 male infertility patients with spleen and kidney deficiency type were collected, all cases were randomly allocated into three groups, group A using methods of invigorating spleen and kidney, group B using Antioxidant therapy, and group C as the blank group. There are 26 cases in each group. One course of treatment lasts for 12 weeks. Before and after treatment, the changes of sperm quality index were analyzed by computer assisted sperm analysis (CASA) and sperm morphology. The pregnaucy outcome after IUI was followed up.Results After the treatment, sperm motility and sperm forward movement (PR) difference before and after treatment in group A with was statistically significant (P < 0.05).The differences between patients with semen volume, sperm concentration, sperm morphology and sperm motility parameters before and after treatment in three groups were not statistically significant (P > 0.05).Clinical pregnancy rate and cumulative pregnancy rate and the live birth rate in group A were higher than that of group B and group C,but the difference was not statistically significant (P > 0.05).Conclusion Invigorating spleen and kidney method can improve the sperm motility and forward movement speed of the type of patients for the treatment of male infertility patients with asthenospermia, and it is helpful to improve the pregnancy outcome of the IUI.

ABSTRACT

OBJECTIVE:To systematically review the efficacy and safety of reinforcing kidney and invigorating spleen princi-ple in the treatment of primary osteoporosis (POP) in middle and elderly aged patients, and provide evidence-based reference for clinic. METHODS:Retrieved from VIP,Wanfang Database,CJFD,Medline,PubMed,Elsevier,Springer,Web of Science and Ovid, randomized control trials (RCT) about reinforcing kidney and invigorating spleen principle (test group) versus other meth-ods(no reinforcing kidney and invigorating spleen principle, control group)in the treatment of POP were collected. Meta-analysis was performed by using Rev Man 5.3 software provided by Cochrane after data extraction and quality evaluation modified by Jadad scale. RESULTS:Totally 28 RCTs were enrolled, involving 2849 patients. Results of Meta-analysis showed, the total effective rate [OR=3.80,95%CI(2.76,5.23),P<0.001],BGP level [MD=2.23,95%CI(1.80,2.66),P<0.001],BMD level of femoral neck [MD=0.04,95%CI(0.00,0.08),P=0.05] and BMD level of lumbar spine [MD=0.07,95%CI(0.03,0.11),P=0.001] in test group were significantly higher than control group,ALP level [MD=-5.59,95%CI(-10.51,-0.66),P=0.03] and VAS score [MD=-1.38,95%CI(-1.87,-0.89),P<0.001] were significantly lower than control group, with statistical significance. There were no significant differences in U-Ca/Cr [MD=-0.07,95%CI(-0.17,0.02),P=0.12] and serum Ca2+ level [MD=0.16,95%CI (-0.07,0.39),P=0.17] in 2 groups. CONCLUSIONS:Reinforcing kidney and invigorating spleen principle can improve patients' osteogenesis during bone metabolism, reduce the activity of the osteoclastic process and relief patients'pain in the treatment of POP.
